Cogent Healthcare provide Care at Home services and Housing support services for adults above 18years and older people over 65 years. These services are provided in clients own homes at agreed times. We accept self-funding or private clients and/social work funded clients who are referred to us and whom we assess and find to be in need of the support service we provide.
Our fully trained Support workers will visit the service users’ homes and provide support according to the person centred care plans provided, working on a rota.
